    Job Description
    SSI/SSDI Outreach, Access, and Recovery (SOAR) is a model that helps individuals who are experiencing or are at-risk of homelessness, who have mental illness and/or co-occurring substance use disorder or other medical impairments to apply for Social Security disability benefits.
    Duties and Responsibilities:
    • Coordinates with referral sources and community partners to identify qualified candidates
    • Provides paperwork as referred to program by filing initial documentation of representation with the SSA office
    • Performs interviews to gather information to complete SSI/SSDI applications
    • Provides medical records and other information to complete SSI/SSDI applications
    • Creates SOAR Medical Summary Reports for applications
    • Coordinates visits to medical doctors, psychiatrists, and other specialists to obtain evidence for case
    • Organizes and participates in appointments at the Social Security Administration
    • Follows Minnesota Data Practices
    • Required to attend any trainings/certifications as required by the State
    • Maintains active client files to ensure program requirements
    • Provides reports as required by funding sources and completes applications for a variety of programs
    • Represents the Agency at community meetings and serves as a liaison between the homeless and local community and asserts public relations efforts
    • Bachelor’s degree in human services/related field
    • At least two (2) years of experience in mental illness, substance abuse, and/or homelessness
    • Experience in conflict resolution, crisis stabilization, and counseling and case management methods
    • Valid driver’s license with a clean driving record
    • Computer proficiency
    Needed Attributes:
    • Ability to work with people from a variety of economic and cultural backgrounds and to work in non-traditional settings and unstructured environments
    • Must be flexible with excellent organizational and communication skills
    • Knowledge of medical and psychiatric terminology, with the ability to write using same
    • Capable of working independently within a team
    • Creative problem-solving skills helpful
